Purpose:
The Library Materials Suggestion Form is distributed in order to provide an opportunity for students and faculty to suggest the addition of a title(s) to the collection. The assumption is that this opportunity will provide more collection development information concerning research resource needs, as well as indicating students and faculty who are not effectively using LUIS.
Timeline:
The process should be completed within a four week period.
Procedure:
The Library Materials Suggestion Form will be distributed at all public services desks, excepting those located in the Health Science Center Library.
Completed forms will be accepted at all public service locations.
Completed forms will be forwarded to the appropriate bibliographer who, in turn, will forward the form to the appropriate selector.

If the title requested is listed in LUIS or in the card catalog, the selector should note the call number and return the form to the requestor if possible.
If the selector checks the shelf for a requested item which is listed in LUIS or the card catalog and finds it missing, the selector should submit a trace form, containing the requestor's name and address if possible, to Circulation.
The selector or bibliographer will decide to select or not select the title and return the request form, if possible, to the person making the request.
