Brittle Book Retrieval Request

Instructions
  1. Items listed in WebLUIS as Brittle Books - Consult Service Desk have been shelved separately in an area not directly accessible by library users to prevent further damage while awaiting microfilming (for long-term preservation of the item in the UF Libraries' collection).
  2. Items requested will be processed within one working day (Monday-Friday) from the date of submission. Notification with details of availability and physical condition (the deciding factor in use policy) of the title in question will be forwarded to the requestor, including where to view/obtain the item.
  3. These items are generally accessible when needed and may be viewed by requesting retrieval using this form
  4. Depending on the condition of the item, pages may be able to be photocopied;some items may also be able to be checked out.
  5. Because of space limitations in the temporary holding area, we must return items to storage that have not been consulted within three (3) working days of notification.
  6. In order to organize these forms to locate items, only one title can be included on each request form.
IMPORTANT: All fields must have contents before submission can occur. If information is unknown, please put N/A or a question mark (?) in the appropriate field. If critical information is not submitted, the request cannot be processed.
